Returning home after a hospital stay can be a vulnerable time. Our hospital discharge support service provides prompt, practical care to help people recover safely at home, rebuild their confidence and reduce the risk of being readmitted to hospital.

We can arrange care at very short notice — often within 24 hours — and work closely with hospital discharge teams, social workers and healthcare professionals to ensure a smooth transition home. Our care plans are reviewed regularly as recovery progresses.
